Driver :

1. Drive Application for Employment.

2. Motor Vehicle Driving Record for MVR.

3. Drive release and authorization to obtain a MVR / PSP.

4. Medical Examiner's Certificate.

5. Background check (written on telephone); 3 to 10 years (CDL).

6. Driver's Road Test & Certification, attachement photo copy of CDL or DL.

7. Driver's Data sheet (previous 7 days) ALL NEW HIRES.

8. Annual list of violations and Annual Review confirm with current MVR.

9. Controlled substance and Alcohol testing history previous employer.

10. Control Substance testing.
